Community Health Needs Assessment

The Affordable Care Act contains numerous provisions aimed at improving community health through direct investments in wellness and prevention at the individual and community levels and by making population health improvement an integral part of efforts to improve the quality and efficiency of health care.

We are pleased to share with you the results of the Community Health Needs Assessment Report for the St. John Valley and Aroostook County. The responses to the survey help us to understand the health needs of our communities and to provide us with the information we need to design and implement strategies aimed at improving the health status of the people we serve. Community engagement using shared Community Health Needs Assessment reports for local and regional planning purposes is a critical part of the needs assessment and health improvement planning process.
