Pharmacists play an important role as an integral member of the healthcare team. The pharmacist responsibilities in a hospital goes beyond dispensing medications. Key functions include assistance in medication prescribing recommendations, monitoring the effectiveness and effects of prescribed medications, counseling and educating patients about medication interactions and safe administration practices. The pharmacist is a member of the patient care interdisciplinary team and has an important role in preparing the patient for discharge from the hospital. The Meds-to-Beds Program supports the smooth transition for the patient back to their home and an important strategy in preventing hospital readmission. With bedside delivery of medications, patients receive a one-on-one visit from a pharmacist who can reinforce education and resolve issues that may otherwise arise after discharge.
