Social Services

Social workers provide frontline services to patients with conditions spanning the entire health care continuum. Social workers are vital members of the interdisciplinary clinical team and use case management skills to assist patients and families address and cope with financial, social and emotional problems that relate to illness or hospitalization. 

Discharge planning is led by social workers and focuses on ensuring a smooth transition for the patient from hospital to home. They serve patients and their families to navigate from hospital to recovery, acting as a liaison between the patient and community resources. They educate patients and their families on entitlements, community resources, and assist in completing health care advanced directives.
